Juggling care coordination and medical bills can be time consuming. As a member of the Sunrise Advantage Plan (HMO I-SNP), Reflections (HMO C-SNP) or Sunrise Advantage Community Plan (HMO), you’ll have the support of a dedicated nurse practitioner led team within your Sunrise community who will take care of the details — leaving you with more worry-free time to spend with your loved ones. You will also have access to a Member Advocate, who will provide you with additional conveniences and support as needed, such as answering your Medicare questions, responding to invoices and managing your network of providers.
Sunrise Advantage Plan is a Medicare Advantage health plan offered through Sunrise Senior Living. Sunrise Advantage Plan is far more than a traditional health plan. It offers a unique model of care involving comprehensive care coordination that is focused on the distinct needs of each of its members, along with the continued input of our Sunrise community care team.
Sunrise Advantage Plan covers everything that Original Medicare covers under Parts A and B, including inpatient and outpatient hospital services, as well as primary and specialty physician care. In addition, it offers Part D pharmacy coverage, and member-centric support and services, to further enhance covered benefits.

What is a Medicare Advantage Plan?
Medicare Advantage Plans are offered by private companies, approved by Medicare, who Medicare pays to cover your Medicare benefits. Medicare Advantage Plans provide benefits covered under Medicare Part A (Hospital Insurance) and Medicare Part B (Medical Insurance).

What is an I-SNP?
Institutional Special Needs Plans (I-SNPs) restrict enrollment to Medicare Advantage (MA) eligible individuals who, for 90 days or longer, require or are expected to need the level of services provided in a long-term care facility or assisted living facility.

What is a C-SNP?
Chronic Condition Special Needs Plans (C-SNPs) restrict enrollment to special needs individuals with specific severe or disabling chronic conditions. Sunrise Advantage Plan is specifically for individuals with dementia.

What is a Medicare Advantage Part D Plan?
A Medicare Advantage Part D Plan is a Medicare Advantage Plan that also provides benefits covered under Medicare Part D (Prescription Drug Insurance). Sunrise Advantage Plan (MAPD) is an example of a Medicare Advantage Part D Plan.
Keep in mind, regardless of the Sunrise Advantage Plan you choose, you will always have exclusive access to the comprehensive coordination, support, and care you need from your Personal Health Team within our communities.

Personal Health Team
As a member of the Plan, a dedicated Personal Health Team is available to you onsite in your Sunrise community. Your Personal Health Team will assist with comprehensive care coordination and help you access exclusive healthcare benefits.
Your Personal Health Team’s services are included in the plan and you will never receive a bill or co-pay from them. The Plan’s Nurse Practitioner is licensed in your state and will work with your PCP to manage your healthcare needs. The Plan’s Nurse Practitioner is available onsite or by phone to help ensure that you have the care you need when you need it.
